fix(ui): replace Title with Text in Modal title to avoid h5 in h2 nesting

Fix:
- Change Modal title from <Title order={5}> to <Text fz='lg' fw={600}>
- Avoids invalid HTML nesting (<h5> cannot be child of <h2>)
- Maintains same visual appearance
