docs: split CLAUDE.md into focused reference files

Move Common Commands to docs/COMMANDS.md and add docs/CONVENTIONS.md
for frontend patterns (SWR, filters, Mantine, routing, API URLs).
CLAUDE.md now only contains runtime rules and pointers.
